• You live, work, worship, attend school or volunteer the Minnesota counties of Anoka, Chisago, Dakota, Hennepin, Ramsey or Washington.
• You have a family member who is a PCU member.
• You work for a PCU Select Employee Group.
• You live in Minnesota or Wisconsin and agree to make a $5.00 donation to the John Miller Scholarship Fund.
